We are now taking bookings for  'Flu vaccinations. Please call our Patient Services Team if you are eligible and would like to book. This year's criteria can be found here.

On the afternoon of Wednesday the 30th September we will be holding a flu clinic at both our surgeries. We will be closed for all other appointments during the afternoon and our phone lines will be closed. There will be an alternative number for patients to call for urgent medical advice and we will publicise this nearer the time.

New Patient Registration

If you live within our practice area we are happy to accept you as a patient. Please see the lovely comments we've received from some of our current patients on our Results of Patient Survey page.

There is quite a bit of paperwork to fill in to become registered and so please pop in to collect a pack for each member of the household who wishes to register or download from the links below. Once you've completed the paperwork send it back to us (drop it in to reception, put it in our letterbox or scan and email) and we'll get you registered. It's very straightforward!

 

Anybody on regular medication will need to have a telephone consultation with one of our practice pharmacists before they can be prescribed more. 

 

Adult Registration pack:

Please fill out all parts of the registration pack once you have downloaded it. We may not be able to register you if you haven't completed all relevant parts.

2. Download and complete the NHS GMS 1 form here

Once both forms have been completed for each patient, they can be emailed to: ibstockhouse@nhs.net

Child Registration pack (Children 0-16 years)

Please fill out all parts of the registration pack once you have downloaded it. We may not be able to register your child if you haven't completed all relevant parts.

Temporary Residents

You can register as a temporary resident if you plan to live near the surgery for up to 3 months.

After 3 months you will have to apply to register as a permanent resident using the adult/child registration pack above.

You can register with the practice temporarily while away from home for work, study or a holiday. You'll remain registered with your permanent GP surgery.

We will pass details of any treatment you have to your permanent GP surgery. They will add the information to your medical records.

Try to have the following information with you when you see a temporary GP for the first time:

  • details of any medical conditions you have
  • details of any medical conditions you've had in the past
  • the name of any medicines you're currently taking
  • details of anything you're allergic to
  • contact details for your permanent or previous GP surgery
  • your NHS number
