Come and see us with two forms of ID* and we can sign you up immediately for online access to order your repeat medications and book telephone consultations via the web.

* We need one item of photo ID and one with your address dated in the last 3 months

There is quite a bit of paperwork to fill in to become registered and so please pop in to collect a pack for each member of the household who wishes to register or download from the links below. Once you've completed the paperwork send it back to us (drop it in to reception, put it in our letterbox or scan and email) and we'll get you registered. It's very straightforward!
Anybody on regular medication will need to have a telephone consultation with one of our practice pharmacists before they can be prescribed more.
